This study explores if a special clinic, called the Cardio-Metabolic Clinic, can better protect heart health in patients with both diabetes and heart disease. The clinic uses a multidisciplinary approach, which means a team works together to look at the whole patient, not just one problem at a time. This clinic aims to improve care by using the latest treatments and heart-protective medicines.

The study compares this special clinic's care with regular care to see if it reduces heart-related illnesses and deaths. Patients will be part of the study for 3 years and will have health check-ups at the beginning and end. After 5 and 10 years, health records will be checked for any complications.

Patients must be over 18, have type 2 diabetes, and heart disease to join. Those with a life expectancy under 5 years or type 1 diabetes cannot participate.
